This guide will assist you in submitting claim via the Employee Self Service (ESS) 


Add New to add a new claim transaction.



Key in the necessary details such as the Description, Date, and Period. Then click on Save to create the claim request.



Once saved, you will be able to see what the options / type are of claims that they can submit along with the entitlements such as Yearly Limits, Monthly Limits, and Per Visit Limits.



Choose the type of claim that you would like to submit and key in the necessary details and click on diskette icon along the claim details to save the claim details you have inputted.


Once done, click on Save at the top right corner to save and submit the claim details. A confirmation message will appear, click on Yes to confirm the submission.




If all criteria / requirements are met then you will be  able to successfully submit the claim.
